Not sure how the "in"s work...

I completely understand how the out points are counted, someone clicks on the link in the toplist, boom, we got 1 point out. But how is the IN counted since the button link is the main page of our toplist? or is the "in" link the unique visits on the site?

No, the "in" is a button/link click on the member website ( unique , once per 24 hours )
The determining of the username who gets the In, is done via http referrers.

On Stat buttons - Hits are the amount of unique visits, or are they "in's"?
What are PVs?
How is it determined who is winning? Or do I have to choose?

On stats button its PV. But can be edited to take hits in by editing settings_button.php

Pv = Pageviews - How many unique pageloads the website has

Winning is determined by who has the highest average or number only ( based on what you choose in admin ) at the end of the ranking period
